Fatal crashes in the US
From 2020 to 2023, 178 LS vehicles were involved in crashes on US public roads in which someone died, and 116 people died inside them. These are counts, not crash rates: a model with more cars on the road shows up more often, and people killed outside the vehicle are not counted here.

New registrations in Europe
27 new Lexus LS cars registered in the EU, Norway and Iceland between 2013 and 2022, from the European Environment Agency's CO₂ monitoring of every new car. In 2022 the largest markets were Croatia (1).
| Year | New cars | CO₂ g/km |
|---|---|---|
| 2022 | 1 | 211 (WLTP) |

Averages are weighted by registrations; CO₂ is the type-approval value (NEDC until the WLTP switch). European Environment Agency, CO₂ emissions from new passenger cars · re-use permitted with acknowledgement
EU safety alerts
Safety Gate, the European Union's rapid alert system, names the LS in 1 alert. European versions can differ from the North American car.
The gasket seated between the fuel pressure sensor and the duel pipe could become degraded. During vehicle operation, fuel could leak past the gasket. In the presence of an ignition source, this could increase the risk of a vehicle fire.

Recalls
7 campaigns cover the LS.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2018-2021 Lexus LS500 and 2018-2020 Lexus LS500 Hybrid vehicles equipped with the "Executive Package." The front passenger seat may have been assembled with interference between internal parts that can cause the Occupant Classification System (OCS) sensor to incorrectly detect the occupant. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023 Mirai, Lexus LS500, LS500H, 2023-2024 Lexus ES250, ES300H, ES350, 2024 Lexus LC500, and LC500H vehicles equipped with a Panoramic View Monitor (PVM) system. Due to incorrect programming of the parking-assist electronic control unit (ECU) software, the rearview camera image may not immediately display within a couple of seconds after the shifter is placed into reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2018 Lexus LS vehicles equipped with run-flat tires with a sidewall reinforcement layer inside the tire. The tire-on-wheel assembly process may have caused the reinforcement layer to crack, possibly resulting in the tire tread layer separating from the tire.
TOYOTA IS RECALLING 214,570 MY 2006-2008 LEXUS IS, MY 2006-2007 GS AND MY 2007-2008 LS PASSENGER VEHICLES EQUIPPED WITH ALUMINUM FUEL DELIVERY PIPES (FUEL RAILS). ON CERTAIN VEHICLES, DUE TO IMPROPER ASSEMBLY OF THE AIR BAG INFLATOR, WHICH IS USED IN THE SIDE AIR BAG, THE CURTAIN SHIELD AIR BAG, AND THE KNEE AIR BAG ASSEMBLY, SOME INFLATORS WERE PRODUCED WITH AN INSUFFICIENT AMOUNT OF THE HEATING AGENTS NECESSARY FOR PROPER AIR BAG DEPLOYMENT. IN THIS CONDITION, THE EXPANSION FORCE OF THE GAS MAY BE INSUFFICIENT TO PROPERLY INFLATE THE AIR BAG WHEN THE SRS SYSTEM IS ACTIVATED DURING A CRASH.
