The issue
THE FOOT CONTROLLED BRAKE VALVE IN THE AUXILIARY OPERATING STATION MAY HAVE BEEN IMPROPERLY PLUMBED (AIR LINES CROSS- CONNECTED). IF THERE IS A SERVICE BRAKE AIR LINE FAILURE IN EITHER THE "A" OR "B" SYSTEM AIR SUPPLY, THE CROSS PLUMBING OF AIR LINES MAY RESULT IN LOSS OF AUXILIARY OPERATION STATION BRAKE CONTROL. THIS COULD RESULT IN A VEHICLE ACCIDENT. CORRECTION: AIR BRAKE SYSTEM WILL BE TESTED AND AIR LINES FOUND TO BE CROSS PLUMBED WILL BE DISCONNECTED AND PROPERLY RECONNECTED.
