Fatal crashes in the US
From 2020 to 2023, 57 Insight vehicles were involved in crashes on US public roads in which someone died, and 28 people died inside them. These are counts, not crash rates: a model with more cars on the road shows up more often, and people killed outside the vehicle are not counted here.

EU safety alerts
Safety Gate, the European Union's rapid alert system, names the Insight in 4 alerts. European versions can differ from the North American car.
A defect in the gas generator of the front-seat passenger airbag leads to uncontrolled inflation and the release of metal fragments when the airbag is activated. This can cause the occupants of the vehicle to be injured.
Due to a defect in the gas generator of the driver's air bag, deployment of the air bag could result in small metal fragments being released which could injure the occupants.
The inflator case component within the airbag assembly may rupture during passenger airbag deployment. In the event of an inflator rupture, metal fragments could be propelled into the passenger compartment, potentially causing injury to a vehicle occupant.
