Fatal crashes in the US
From 2020 to 2023, 921 Pilot vehicles were involved in crashes on US public roads in which someone died, and 383 people died inside them. These are counts, not crash rates: a model with more cars on the road shows up more often, and people killed outside the vehicle are not counted here.

EU safety alerts
Safety Gate, the European Union's rapid alert system, names the Pilot in 8 alerts. European versions can differ from the North American car.
Due to a faulty circuit board in the passenger seat weight sensor, the passenger seat’s occupant detection system may identify a Steering Wheel Sensor (SWS) error and the Supplemental Restraint System (SRS) warning light could switch on. This can cause a fault…
The brake master cylinder may detach, affecting the braking performance and creating a risk of an accident. The product does not comply with the Regulation on the approval and market surveillance of motor vehicles and their trailers, and of systems, components…
At high speeds, the air inlet between the bonnet and the radiator grill causes vibrations in the hood. This can result in stress fractures along the bonnet latch striker and its separation from the bonnet, causing the bonnet to open. This obstructs the drivers…
A defect in the gas generator of the front-seat passenger airbag leads to uncontrolled inflation and the release of metal fragments when the airbag is activated. This can cause the occupants of the vehicle to be injured.
A fault with the inflator of the front passenger airbag results in the release, when the airbag is activated, of small metal particles which may injure the occupants of the vehicle.
The inflator's propellant may degrade over time due to heat cycles and exposure to moisture. In case of an accident with airbag deployment, the inflator may burst and metal fragments may be expelled and cause serious or fatal injuries.
Owing to excess internal pressure, inflation of the driver airbag can cause the inflator to split and release metal fragments which can cause injury to the vehicle passengers.
A faulty component in the electronic VSA (Vehicle Stability Assist) system can cause a brief application of the brakes. If the driver brakes when such a fault occurs, the VSA system may activate the braking assistance function, thus boosting the braking effect…
